Goncharuk O. Microsurgery of damages peripheral nerves of the bottom finitenesses in conditions, adverse for regeneration of a nerve (clinical-experimental research).

The dissertation is an experimental-clinical research and is devoted to microsurgery of damages of peripheral nerves of the bottom finitenesses in unsatisfactory conditions for regeneration. It is selected and developed new direction in a morioplasty, namely, microsurgery of damages of peripheral nerves of lower extremities subject to the condition unsatisfactory for their regeneration. Protectors possibilities of fatty fabric and developed technique of delete, preparation and transplantation of fatty fabric studied in the experiment in the area of interference on a nerve. First in medical practice it is substantiated for the selection of extremity as organ with the special, inherent only to him by biomechanics, haemodynamic and trophic descriptions. On experimental models the reproduced unsatisfactory conditions (a chronic compression, purulent process; a venous hypertensia, a chronic ischemia, consequences of a diabetes) for regeneration of a buttock nerve which has been connected perineural by a seam.- The morphological results Studied in experiments, have allowed to define the most influential negative factors and to transfer the received experience to clinic. Are operated 244 patients for which treatment modern methods of diagnostics are used, opportunities of microsurgery of nerves, surgeries of vessels. Results statistically authentic are received in experiment and clinic.
