Essay

The thesis contains a new trend in surgical treatment of bone shaft fractures and their consequences by developing new techniques of osteosynthesis. Author has designed a new device and technique for biomechanical testing which provides any given simple loadings and can be used for comparative analysis of different bone fragment fixation technologies in different types of fractures. Mathematical apparatus of materials resistance theories and structural mechanics was adapted for implementation in osteosynthesis tasks, and used to substantiate the shape and size of implant corpse, the number and diameter of interlocking screws. A new technique of measuring spongy bone macrostiffness was developed and used for proximal metaepiphysis of the femoral bone, which provided necessary data for calculating fixation stiffness at the contact area between spongy bone and implant. Advantage in fixation efficiency in “bone-IMPN” system was biomechanically substantiated for femoral and tibial diaphyseal fractures. Newtechniques were designed and substantiated for interlocking nailing of femoral, tibial, and humeral shaft fractures with IMPN-2, IMPN-3, IMPN-6, IMPN-8, IMPN-3H. Approbation of designed techniques in surgical treatment of 682 patients with bone shaft fractures and their consequences in clinics of Bukovinian State Medical University showed their high efficiency that makes good to recommend the techniques for wide implementation in practical health care
