Dissertation is devoted to study the increase of efficacy of early diagnostic of primary antibody deficiency in children by determination of early diagnostic criteria and improvement of differential approach to the treatment of primary immunodeficiency in children. The most frequent clinical signs in 120 children with primary antibody deficiency are recurrent bacterial infections, mostly of respiratory system, with high frequency of complications and progression to chronic changes. Diagnostic significance of different clinical signs and symptoms as indicators of early diagnostic of primary antibody deficiency was studied. The diagnostic algorithm of primary antibody deficiency was created on the basis of evaluation of immune status and genetic markers in children with different forms of immunodeficiency. Clinical and immunological effectiveness of regular replacement therapy by intravenous immunoglobulins is proved in children with severe hypogammaglobulinemia. The efficacy of antibacterial prophylaxisof recurrent infections is proved in children with primary antibody deficiency. The differential approach to the vaccination of children with different groups of primary antibody deficiency is determined. The clinical registry of primary antibody deficiency is created.
