Essay

Based on a complex examination of 360 adolescents aged 15-18 with functional digestive diseases, pathogenetic aspects of formation of these disturbances, variants of their clinical course were revealed and a system of rendering of medical and preventive assistance to adolescents with functional digestive diseases was developed. Findings of the performed studies show that functional digestive disorders are closely associated with the status of the vegetative nervous system and are a manifestation of vegetative disturbances. The latter develop in most patients with underlying sympathicotonia and the vegetative reactivity manifest itself as hyper- or asympathicotonia. The role of neuroendocrinal mechanisms in formation of main syndromes of vegetative disfunctions and functional digestive disturbances in adolescents is studied. The role of neurotransmitters and adenotropic hormones in formation of borderline types of personality accentuation, vegetative disorders and non-organic diseases of digestive system is proved. Work findings show that the pituitary dominant in adolescents is accompanied with hyperfunction of adenotropic hormones, in particular, thyroid-stimulating hormone. Based on study of the functional status of the hypothalamic-pituitary-gonadal axis and thyroid system and correlation between single elements of these systems, the role of hormonal disorders in the genesis of non-organic digestive diseases is established. On the basis of complex study of neuroendocrinal and psychovegetative homoeostasis findings in adolescents with functional digestive disorders, a differential complex of therapeutic measures is developed and data on higher efficacy of the proposed therapy are obtained.
