Orlov M. Clinical manifestation, diagnostics and treatment of arterial aneurysms, arteriovenous and venous malformations of brain at children.

The dissertation is devoted to a problem of topographo-anatomic, structural-morphological, angiodynamic features of brain vessels development anomalies (arterial aneurysms (АА), arterio-venous (АVМ) and venous (VМ) malformations) at children, to their clinical manifestation, diagnostics and treatments results improvement. The research is based on the analysis of 381 clinical cases. 72 АА, 253 АVМ, and 56 VМ were diagnosed at these patients. The increase of patients' amount is clearly traced with the increase of their age, regardless of vascular anomalies forms.Sharp beginning of the disease was marked at 82.4% supervisions. At АVМ sharp beginning of disease clinical manifestation was established in 84.9% cases, at АА - in 79.1%, at VМ - in 75%.Modern possibilities of brain vascular malformations instrumental diagnostics are wide enough.Treatment measures at brain vascular defeats at children included open transcranial microsurgical interferences, endovascular operations, radiosurgery and their combinations.Postoperative lethality was 0.6% (at АА - 1.4%, at АVМ - 0.4%, at VМ there were no lethal cases). The patient's state improvement after operation was established in 57.2% cases, without changes - in 36.3%, state worsening - in 5.9%. The remote results analysis of brain vascular anomalies treatment at children was carried out on material of 317 supervisions. Good quality of life was marked at 56.5% of all children after different types of surgical treatment.
