Essay

The thesis is devoted to the problem of treatment results improving in patients with acute brain injury and temporal bone pyramid fracture (TBPF) through working out and perfection of complex diagnostics, clarification of certain pathogenesis factors that influence on clinical course and determaination of treatment tactics. The informative value of TBPF clinical signs and neurovisualising methods in it's diagnostics and verification at acute brain injury were defined. The reliable distinctions were defined for longitudinal and transversal TBPF by clarification of typical clinical symptoms complex that identify features of disease course. The correlation between type of TBPF, intracranial damage (by computer tomography) and severity of brain injury (according to Glasgow Coma Scale) was revealed. An important role of brainstem auditory evoked potentials time parameters was proved for clearing of brain compression and dislocation process and prognosis of clinical course of acute brain injury with TBPF. The complex of computer stabilography diagnostic criteria for statocoordination disorders exposure in patients with acute brain injury and TBPF was proposed. On the base of obtained data the standardized variant of clinical and instrumental otoneurological examination of patients with acute brain injury was worked out that increases TBPF diagnostics efficacy and makes it objective. Optimal complexes of diagnostic and treatment measures at otoliquorrhea and facial nerve peripheral lesion in patients with acute brain injury and TBPF that promote improving medical aid quality and efficacy were proposed
