Object of investigation - rheumatoid arthritis and ankylosing spondylitis with esophageal complications; aim - study of pathogenetic significance immune, metabolic, trace element and salivary disorders in causing esophageal complications in patients with rheumatoid arthritis and ankylosing spondylitis, and development of a complex treatment with the esophageal protection on this basis; methods - clinical, biochemical, immunological, enzyme-linked immunosorbent assay, instrumental, morphological, statistical; newness and results - the nature and structure of the esophageal complications in patients with rheumatoid arthritis and ankylosing spondylitis were first detected; interrelation between salivary dysfunction and esophageal failure in these patients was established; features of the morphological changes of esophageal mucosa in patients with rheumatoid arthritis and ankylosing spondylitis were first identified; effect of microelement imbalance and anemia on the nature and severity of esophageal lesions were detected; pathogenetic significance of immune disorders, cytokine imbalance and overproduction of nitric oxide in patients with rheumatoid arthritis and ankylosing spondylitis in the mechanisms of formation of oesophageal disease was first found. Influence of gastroesophageal reflux on the quality of life in patients with rheumatoid arthritis and ankylosing spondylitis was assessed; complex treatment with the esophageal protection therapy of rheumatoid arthritis and ankylosing spondylitis was theoretically justified; its influence on the formation of osteopenic syndrome in these patients was evaluated; introduced - in practice of Scientific-practical medical center of Kharkiv National Medical University, Kharkiv Clinical Hospital №1, Educational and Scientific medical complex of Donetsk National Medical University, Institute of Urgent and Recovery Surgery of NAMS of Ukraine, Rheumatological center of Donetsk Regional Clinical Territorial Medical Association, Poltava Central District Hospital, Lugansk Regional Veterans Hospital them.
