Essay

The dissertation deals with a study of the clinico-paraclinical characteristics of the course of duodenal peptic ulcer in children, ascertaining the role of genetically determined mechanisms of its development with an elaboration and substantiation of differentiated diagnostically prognostic and treatment algorithms for the purpose of enhancing efficiency the diagnostics, prognosis and treatment of the disease. The epidemiology of the pathology under study has been researched in children of the Chernivtsi region, the genes of pathogenecity H. pylori, the character of changes of the oxidant-prooxidant and interleukine homeostasis has been analyzed, depending on the presence and toxigenicity of H. pylori and the polymorphism of the genes, of interleukines, their impact on the development and severity of the disease course having been corroborated. We have also isolated the phenotypes of duodenal peptic ulcer, established the predictors of the risk of the development, the severity of the course and the recurrence rate, mathematical models of the risk of the development and severity of the disease course have been elaborated, a necessity of using the genotype-specific therapy has been suggested and corroborated for the first time.
