Essay

The dissertation deals with the investigation of the problem of gastric mucosa (GM), defining its morphogenesis with the grounding of new approaches to the principles of the early diagnostics and optimization of the curative and preventive tactics. As the result of the performed investigations there has been established the high frequency of combination of metaplasia and atrophic changes in GM in chronic atrophic gastritis, which substantiates the rightfulness of singling out a separate type of atrophy - a metaplastic one, and, consequently, chronic atrophic metaplastic gastritis. There has been determined the leading role of the H.pylori infection in the development of metaplastic, atrophic and dysplastic changes in GM. There have been developed the endoscopic, histological, histo- and immunohistochemical criteria for the early diagnostics of intestinal metaplasia (IM), which became the basis for defining the IM morphogenesis with the consecutive stages of structural and functional transformations in GM (reprogramming of the genetic material of the gastric epithelium nuclei, histochemical differentiation of the metaplastically changed epithelium, consolidation of the phenotype with progressing), development of the algorithm of the pathomorphological diagnostics of IM and the screening program for the optimization of the curative and preventive tactics for patients with precancerous conditions of GM.
