Essay

The dissertation is dedicated to the problems of improving of the efficiency of prophylaxis and treatment of the pre- and postnatal iodine and iron deficiency disorders in infants. The 30-cluster survey in pregnant and infants of Luhansk Region was conducted. Insufficient iodine status in pregnant was demonstrated: median of urinary iodine concentration - 78.2µg/L, frequency of goiter in pregnant - 72.8% and neonatal hyperthyrotropinemia in newborns as a results of regional screening for congenital hypothyroidism (20.6%). Median urinary iodine concentration in lactating mothers was 91.2µg/L, median breast milk iodine concentration - 68.0µg/L. Median urinary iodine concentration in breast-feeding infants was 81.2µg/L, in formula-feeding infants - 282.0µg/L. The existence of the iodine deficiency disorders and iron deficient anemia in infants was proved to be the health and social problem in the region. It was demonstrated that the low frequency of individual iodine prophylaxis during pregnancy and lactation is the main etiological factor of iodine deficiency disorders in infants. It was found out the high level of minimal hypothalamic-thyroid dysfunction in infants at the region. It was proved that hypothalamic-thyroid dysfunction is closely connected with iron deficiency. It was proved that the individual iodine prophylaxis is more effective than iodine salt intake in pregnant and lactating mothers in mild iodine deficiency region. The conclusion of the necessity of the iron supplementation in breastfed infants was made. The set of individual iodine prophylaxis in pregnant, lactating mothers and breastfed infants was created.
