Taktashov G. Respiratory dysfunction in chronic rheumatic heart disease: the link with ecology, diagnosis, pathogenesis, treatment

The object of study is the course, pathogenesis and treatment of respiratory dysfunction in chronic rheumatic heart disease (CRHD); the aim is to improve the quality of early diagnosis, the efficacy of treatment and prognosis of the course of respiratory pathology in CRHD, to establish new links in the pathogenesis of disorders of ventilation, moisture separating, diffusion, conditioning and surfactant generating lung function in such patients; the methods are clinical, hygienic, electrophysiological, ultrasound, biophysical, physicochemical, biochemical, immunoenzymometric, atomic absorption, statistical; the novelty and the results -the link of cardiorespiratory parameters in CRHD with the levels of certain xenobiotics in exhaled air of the patients has been first proved, the impact of the evolution of juvenile rheumatism on the subsequent nature of the changes of ventilation, moisture separating, diffusion, conditioning and surfactant generating lung function in adult patients with CRHB has been determined, the connection between particular parameters of breathing with the nature of heart disease, functional class of heart failure, the rates of the progression of the disease, performed operations on surgical correction of the disease and with the peculiarities of the hemodynamic disorders in the pulmonary circulation has been first identified, the pathogenic role of the state changes of lipid metabolism, lipid peroxidation, antioxidant defense system, physicochemical adsorption-rheological properties of the blood, endothelial function of blood vessels, the balance of cardioprotective and cardiotoxin of microelements in the formation of the course in CRHD and disorders of lung functions has been primarily established, prognostic criteria of the course of the pathological process and the effectiveness of remedial measures have been emphasized; they have been introduced into practice of SI "NSC "N.
