Essay

Object: the arthrosis of the sacroiliac joints. Aim: to improve the diagnosis, establish criteria for predicting development of the arthrosis of the sacroiliac joints, to develop a system of rehabilitation treatment on the basis of the improved concept of sano- and pathogenesis of the disease. Methods: clinical; radiometric; conceptual and mathematical modeling using the finite element method; electrophysiological methods of examination of patients (superficial electromyography of muscles), statographic; statistical - descriptive statistics, multiple correlation analysis, consistent prognostic procedure. The concept of genesis of the arthrosis of the sacroiliac joint was first developed. For the first time the features of passive stabilization of the sacroiliac joint at the pelvis and sacral base tilt, asymmetry of the SIJ gap width in the frontal plane and various variants of lumbar lordosis in the sagittal plane were investigated using mathematical modeling. The use of the developed concept of sano- and pathogenesis of the arthrosis of the sacroiliac joint will allow to create a new correct idea of the mechanisms of its development and course. For the first time the occurrence of such functional deformations of the pelvis as "twisted" and "tilted" pelvis is mathematically proved. Pathogenetic and clinical-radiological classification of the arthrosis of the sacroiliac joint was first developed. Radiographic criteria for predicting the course of the SIJ arthrosis have been determined for the first time. The use of developed prognostic criteria in clinical practice will provide physicians with a reliable prediction of the disease course with optimization of therapeutic tactics. Creating a pathogenetically bound system of rehabilitation will improve the treatment outcomes of this number of patients. X-ray parameters of the sacrum, pelvis, and lumbar lordosis were studied for the first time in patients with SIJ arthrosis using functional X-ray. The results of the study were introduced into the educational process of the Department of Traumatology and Orthopedics of the Kharkov Medical Academy of Postgraduate Education of the Ministry of Health of Ukraine, in the clinical practice of the SI "Sytenko Institute of Spine and Joint Pathology National Academy of Medical Sciences of Ukraine", Municipal non-profit enterprise "City Clinical Hospital of ambulance and emergency medical care named after prof.
