Turchin O. Diagnostics and Treatment of Plantar Enthesopathies

The dissertation is aimed at solving the actual problem of improving the results of treatment of patients with рlantar еnthesopathies by improvement of diagnostics and treatment, development of system of prediction of results of treatment. On the basis of histological examination it was established that the plantar plate and the area of attachment of the plantar aponeurosis to the calcaneus morphologically correspond to fibro-cartilage enthesis; plantar enthesopathies are characterized by the same type of morphological changes characteristic of fibrocartilage entesopathies. On the basis of clinical and instrumental studies, correlation dependencies and causal relationships between risk factors and plantar enthesopathies were established; improved instrumental diagnostics of plantar enthesopathies. The scheme of differential diagnosis of plantar fasciitis and metatarsalgia has been developed; algorithm of conservative treatment of plantar fasciitis and surgical treatment of metatarsalgia were developed; a system for prognosis long-term outcomes of treatment was created. The application of the developed system of prognosis, diagnostic and therapeutic algorithms allows to improve the effectiveness of treatment, to improve the static-dynamic function of the foot, to reduce the percentage of complications and to improve the quality of life of patients of the selected category.
