Essay

Object - inflammatory diseases of the respiratory organs in children; aim – to optimize diagnostics and treatment of the inflammatory respiratory diseases in children of different age using non-invasive diagnostic tools to implement the tactics of personalized therapeutic and preventive measures; methods – clinical, biochemical, immunological, cytomorphological, histochemical, cytogenetic, microbiological, virological, allergological, instrumental, statistical; it is shown for the first time, that suggested non-invasive diagnostic tools allow to determine the activity and nature of the inflammation in the upper and lower airways in an accessible, informative and objective way; the biomarkers associating with the risk of severe bronchitis, pneumonia, alternative phenotypes of bronchial asthma and their comorbidity with chronic tonsillitis in children were determined based on the indicators of exhaled breath condensate, sputum and its supernatant, nasocytogram, and exfoliative cells of the buccal epithelium; the modified complex clinical-instrumental scoring scale for the assessment of the risk of severe pneumonia was designed and consisted of anamnestic, clinical, instrumental and laboratory indicators including obtained from non-invasive diagnostic tools; the algorithms for personalized therapy of bronchitis, community-acquired pneumonia, and individual phenotypes of bronchial asthma in children were designed through implementing the results of non-invasive diagnostic methods in existing guidelines and protocols, which allows optimizing personalized therapy of these diseases in children.
