Approaches to surgical treatment of postoperative ventral hernias are well studied. However, in some patients, the results of treatment remain unsatisfactory due to the development of treatment complications. Surgical trauma and the presence of the implant in the wound initiate a local inflammatory process, the development of nonspecific inflammatory reactions such as "reactions to a foreign body" that prevent optimal integration of the implant in the tissues. The peculiarities of the tissue reaction, the nature of inflammatory and regenerative processes in the area of implantation of mesh prostheses, and their differences under conditions of local use of platelets rich plasma were studied in laboratory animals. The benefits of using platelets rich plasma have been demonstrated by improving neoangiogenesis and less fibrotization of the area around the prosthesis. The clinical part of the study with 102 patients demonstrated the benefits of local use of platelets rich plasma by improving short-term treatment outcomes such as reducing the number of cases of fluid accumulation and its severity. Also in the remote period there is a decrease in the intensity of chronic pain and an improvement in quality of life is found.
