The dissertation is devoted to solving an urgent scientific problem - improving perinatal consequences in women who need preinduction of childbirth by determining the pathogenetic significance of relaxin and immunological factors in the process of preparing the body for childbirth. Thus, we conclude that in the process of preparing the body for childbirth, relaxin promotes "maturation" of the cervix by regulating the mechanisms of intra- and intercellular interactions. The results of the study allowed to expand modern ideas about the mechanisms of preparation of the body for childbirth, as well as to select effective methods of preinduction of childbirth. The use of comprehensive pathogenetically characteristic treatment has improved the results of preinduction of labor in primiparous women. The method is effective for full "maturation" of the cervix and creating conditions for natural childbirth. The scientific novelty of the study is expands scientific ideas about the features of history, somatic and gynecological status, pregnancy and childbirth at different degrees of "maturity" of the cervix. Clinical and anamnestic risk factors for the formation of "immature" cervix were identified. Scientific concepts about the pathogenetic role of relaxin in the formation of biological readiness for childbirth have been expanded. Based on the expansion of ideas about the pathogenesis of the formation of the "mature" cervix, a modern comprehensive method of preinduction of childbirth has been developed. The scientific data on complex preinduction of childbirth in pregnant women with "immature" cervix are supplemented, the effectiveness of the proposed method is determined, the dynamics of cervical "maturation" dynamics is assessed against the background of traditional and complex preinduction. Developed and proposed in clinical practice a new method for predicting the "maturation" of the cervix in preparation for childbirth based on the determination of relaxin in the serum of primiparous women at 40 weeks of gestation (Patent of Ukraine № 145470 from 10.12.2020, "Method of predicting biological readiness for childbirth").
