VOVK O.Ya. Medico-social substantiation of the functional-organizational model of early detection and prevention of visual forms of malignant tumors: cervical cancer and breast cancer. – Qualifying scientific work on the rights of the manuscript.
The purpose of the work was the medical and social substantiation and development of a model for the early detection and prevention of visual forms of malignant tumors: cervical cancer and breast cancer and the implementation of certain elements of the model in the sphere of health care of Ukraine.
The scientific novelty of the obtained results is that for the first time in Ukraine:
• Medical and social substantiation of the functional-organizational model of early detection and prevention of visual forms of malignant tumors - cervical cancer and breast cancer - was carried out, the key innovative elements of which were: a medical information system with built-in artificial intelligence, which unites all health care institutions a single network; a woman, as a part of the ecosystem in which she is located, and as a subject of management of this system.
• The role of a public health specialist, who is responsible for monitoring and assessing the state of a woman's health, is defined. Thanks to our proposed model, the communication function between doctors of all levels, health care specialists is improved, and a comprehensive approach to the state of a woman's health from different levels of medical care is implemented, ensuring continuity.
• The regional and all-Ukrainian peculiarities of the trends in morbidity, prevalence, and mortality due to cervical cancer and breast cancer have been clarified.
• Modified determinants of the impact on the occurrence of cervical cancer and breast cancer were revealed.
Gaps in the regulatory and legal framework regarding the prevention and early detection of cervical cancer and breast cancer in the state, economic, scientific, organizational, and educational sectors have been identified.
• Expanded understanding of the factors that prevent screening by a woman: logistical, informational, psychological, organizational.
• The role of a man in the early detection and prevention of breast cancer and cervical cancer in women is determined.
Scientific and methodological approaches to the study of models for early detection and prevention of visual forms of malignant tumors have gained further development: cervical cancer and breast cancer.
Improved:
• methods of women's adherence to preventive examinations and its availability;
• scientific and methodological approach to the implementation of the model of early detection and prevention of visual forms of malignant tumors: cervical cancer and breast cancer.
An integrated approach to the organization of a model for early detection and prevention of visual forms of malignant tumors has gained further development: cervicalcancer and breast cancer.
The practical significance of the obtained results is that:
• the research results are the basis for creating a qualitatively new approach to the early detection and prevention of visual forms of malignant tumors: cervical cancer and breast cancer;
• development of an innovative set of measures for the early detection and prevention of cervical cancer and breast cancer through the inclusion of a separate module for a public health specialist and an application for a patient in the medical information system;
• the achievements of the dissertation allow us to use a logical model for the implementation of early detection and prevention of visual forms of malignant tumors: cervical cancer and breast cancer with the definition of its criteria and indicators in order to evaluate the effectiveness of the model;
• organizational tools for the implementation of the model in the practice of specialists are defined: a chain of approval of changes that will allow to identify risks in the system of early detection and prevention of cervical cancer and breast cancer;
• addition of educational programs of public health, social medicine, oncology, family medicine on issues of prevention and early detection of cervical cancer and breast cancer;
• introduction of questions on early detection and prevention of visual forms of malignant tumors: cervical cancer and breast cancer into the educational programs of training doctors at the pre- and post-graduate level.
