Essay

The dissertation covers the issues of the increase in efficacy of the surgical treatment in the patients with the chronic mucous middle ear otitis (CMMO) by way of introduction of new effective methods of the surgical treatment and determining the indications for their application. The research is using a new direction in the surgical treatment of CMMO based on prolonged endaural drainage of the anthromastoid cavity. The surgical methods - of endaural drainage of the aditus and double drainage of the middle ear - were described along with the detailed indications for their application eligibility. The results of the study allowed to determine anatomic indicators for the optimal location of the aditus drainage tube. The determining factors for the choice of the surgery mode were the data received from otomicroscopy, nature of exudate and the degree of the aditus permeability. The high efficacy of the surgical interventions with endaural aditus drainage was proved objectively. Positive clinical, anatomic and functional results were received in 88.0% of the patients after the endaural drainage of the aditus, while 91.9% responded positively after the double drainage of the middle ear and 73.8% - after the shunting of the tympanum (control group).
