Essay

The thesis is devoted to a problem of improvement of diagnostics and complex therapy of juvenile rheumatoid arthritis (JRA) basing on detailed investigation of state of lipid metabolism and immunity and correction of their disturbances by means of inclusion of -3 unsaturated fatty acids (PUFA). Participation of exoantigens in the etiology of separate clinical variants of disease was verified by the disorders of cell immunity characterized by increase of CD4+ cells, activated T-lymphocytes and contents of subpopulation of -T+ cells. It was established, that in URA patients exists both dislipidaemia alongside with the changes of lipid structure of erythrocyte membranes with increase of the contents of total cholesterol, cholesterol/phospholipid ratio and also with the reduction of quantity of free arachidonic acid and increase of its derivatives (eicosanoids)- tromboxane В2 and leucotriene С4. Close connection of disturbances of lipid metabolism with alterations of immune reactivity of children w ith JRA was established. Expediency of inclusion of -3 PUFA in complex therapy JRA was scientifically justified. It'd been proved the high clinical efficiency of including to routine protocols of JRA therapy the drug with high -3 PUFA content -"Thecom", and also capability of it's usage for prophylaxes of relapses of disease.
