42 patient's with aseptic endoprosthetic instability of the hip joint. Aim: To find the biomechanical factors causing early aseptic endoprosthetic instability of the hip joint and to work out the precautionary measures, diagnosis and treatment. Methods of investigation - clinical, radiological, biomechanical, statistical. With the help of finite element analysis, it has been proved that subchondral plate of acetabulum is very necessary for uniform transformation of weight bearing forces of the hip joint and to protect the cancellus bone from the irregular fоrces as a result of overweight bearing. Optimal position of the acetabulur component and femoral stem have been established. It is very necessary to fill the bone defects of acetabulum and femur in reendoprosthesis. Role of subchondral plate in fixing the acetabulur component has been proved. The main clinical, radiological, biomechanical features of endoprosthetic instability have been established, which helps in improving the diagnosis, measuresf or preventing the endoprosthetic instability and methods of surgical treatment with the help of allobonegraft's hydroxylapatite ceramics and bone cement. Results of investigations are been used in Kharkov research Institute of orthopedics and traumatology named after prof.M.I.Sitenko and Kherson Regional hospital. Traumatology and orthopedics.
