Shevchenko V. Surgical treatment of postinfarct aneurism of left ventricle (PIALV).

Immediate and late results of surgical treatment of 108 patients with an IHD> complicated by PIALV, were studied, basing on complex clinic and instrumental investigation using data obtained by electrocardiography, stress-test, ultrasound examination of the heart and roentgencontrast coronaroventriculography. Postekstrasystolik pcxentiation of contraction during performance of ultrasonic and roentgencontrast investigation of heart, echocardiographic test with dobutamin were applied for determination ofthe vital myocardium borders in PIALV. Method of surgical treatment of patients was optimized, the method of determination of royocardium revection was elaborated, new methods of coronary bed reconstruction were proposed, some stages of traditional operative procedures were modified with elaboration and improvement of technical deviels for their conduction, basing on estimation of central and intracardial hemodynamic indexes and determination of myocardium vital capacity. The methods elaborated have permit ted to improve significantly the possibilities of surgical revascularization of myocardium in most severe course of THD with affection of diatal portions of coronary arteries, to shorten the number of contraindications for the PIALV surgical treatment and improve its results.
