The thesis is devoted to the elaboration of the algorhythm of using computer tomography in diagnosis, planning and evaluation of the efficiency of endomicrosurgical interventions in patients with epidural and subdural hematomas. The study is based on the results of the examination of 145 patients, aged from 18 to 75 years. Subdural hematomas predominated - 124, epidural hematomas - 21; subacute hematomas were noted in 45 cases, chronic - in 100. Conventional surgical interventions were performed for 75 patients, in 20 patients hematomas were removed by burr-hole method and in 50 - biportal or multiportal endomicrosurgical method. CT-examination was carried out when admitting in all 145 patients. The projection of the border of hematoma was marked on skin of patient's head effective performing endomicrosurgical interventions. CT-signs of uncomplicated course of postoperative period after removal of subacute and chronic epidural and subdural hematomas were istablished: lack of blood in the bed of removed hematoma, clear differentiation of gray and white matter of the brain with distinctive CT-densitometric indices of their density, smoothing out the brain and the spaces containing liquor; clear image of convexital subarachnoid spaces. CT permits to state surely practically the development of pneumocephalus, repeated hemorrhage, edema and swelling of the brain, intracranial infectious processes. The carrying-out of CT on the 1,5,10 day after surgical intervention in number of 3 scans at the level of lateral ventricles and the bed of removed hematoma is enough for dynamic control of the state of brain and liquor spaces .
