100 patients with the closed and open fractures of the shin bones. The purpose is to develop an apparatus for reposition and external fixation on the base of rods and to carry out experimental - biomechanical and clinical investigations of the proposed method of the treatment of shin bones' fractures with using of the apparatus. Methods of research: informative, biomechanical, clinical, radiological, statistical. For the first time new knowledge about peculiarities of loading distribution in the system "apparatus - fragments" and about value of fragment transferences during loading of injured extremity. It is established that the apparatus receives part of efforts under condition of axis loading, the other part is received by the fragments. Their correlation (the coefficient of rigidity) quantitatively describes the reliability of fragment fixation and degree of their participation in the execution of carrying function during functional loading of injured extremity. The technique of definition of factor of rigidity can be used for an estimation of fixing opportunities of other apparatuses which are developed for the treatment diaphisar fractures. For the first time the technique osteosynthesis for treatment of the shin bones fractures is developed with the help of the apparatus (the patent of Ukraine № 30750) and its modifications (patents of Ukraine № 38702А and № 38397А) which provide an opportunity of elimination of all kinds of displacement of fragments and provides an opportunity of functional loading of the injured extremity. Rational schemes of configuration of the apparatus and technique of its use are developed, depending on concrete clinical situation. By the biomechanical researches there were established the basic parameters of rigidity, angular deformation and safe loads at different combinations of smooth and thread rods. Results are used in clinical practice of traumatological departments in Zaporozhye (9-th clinical hospital of Zaporozhye, regional center of urgent medicine and first-aid, regional clinical hospital, children regional hospital).
