Dissertation is dedicated to the development of methods of differential diagnosis of patients having fever of undefined genesis (FUG), particularly angiocholitis and cholangitis complicated by sepsis, as well as to rational methods of treatment. It was ascertained that 23,4% of patients which were admitted to the clinic with the diagnosis FUG had focus of infection in the biliary system. The key diagnostic methods were duodenal probing in order to obtain all portions of bile, identification of pathogenic agents and their response to the antibiotics which are accumulated in the biliary system.A new method of differential diagnosis of inflammatory processes in biliary system has been developed/ Treatment by sensitive antibiotics, which accumulate in biliary system, was carried out in the majority of the cases according to method protected by a respective patent, which was developed in the clinic. In case the treatment was not effective after the first course of antibiotic therapy, repeated examinations including duodenal probing, identification of causative agent and determination of their sensitivity were made.Thesis presents that by applying such methodology for diagnosis and treatment of biliary sepsis patients the therapeutic success has been already achieved at nearly 54.1% cases during the first course of treatment. To reach full therapeutic success repeated treatment was carried out.
