Essay

The dissertation is devoted to the study of the topical issue of modern internal medicine – optimization of the diagnostics of cardiovascular remodeling in patients with essential hypertension after the early recovery period of hemorrhagic stroke based on a comprehensive instrumental examination of patients, assessment of plasma hemostasis and disorders of lipid metabolism, taking into account pathogenetic treatment. The assessment of the structural and functional state of the cardiovascular system in patients with hypertension (HT) complicated by hemorrhagic stroke after an early recovery period, allowed to clarify the features of remodeling of the major cervical arteries, to establish a correlation with higher detection of atherosclerotic plaques, wider ranges of blood pressure variability and growth of the pulsation index in the major cervical arteries. The predominance of the vascular remodeling over the cardiac one has been determined. The increase in the variability of daytime blood pressure in the systolic component with a slowing of hemocoagulation processes increases the cerebrovascular risks. To identify an increased risk of recurrent hemorrhages at the stages of the recovery period of hemorrhagic stroke in patients with HT, a diagnostic algorithm has been developed to assess the most informative indicators of the spectrum of plasma hemostasis. The use of fixed combination of antihypertensive treatment with rosuvastatin showed clinical efficacy in 72,7% of patients.
