Essay

The dissertation summarizes the results of a comprehensive study, which included the study of anthropometric parameters, characteristics of the clinical course, disorders of the lipid spectrum of blood, clinical and biochemical markers of liver fibrosis, determination of IV collagen and transforming growth factor beta, liver elastography in overweight and obese patients with posthepatitis fibrosis (HCV) and NAFLD, as well as a new solutions for diagnostic optimization and options for correction, predict the course of liver fibrosis by developing a model for assessing the risk of disease progression, justification for treatment of this comorbid pathology. It has been shown that the leading factors in the progression of posthepatitis fibrosis and fibrosis in patients with NAFLD are disorders of lipid metabolism, which correlate with body weight, steatosis and stages of fibrosis. It was found that the degree of liver fibrosis in patients with posthepatitis fibrosis after treated HCV infection and in patients with NAFLD increases with increasing BMI. The most important correlative indices for FIB-4, NFS, Fibrotest and pSWE-elastography in patients with different body mass indices have been identified. Scientific data on the clinical and pathogenetic role of collagen IV and transforming growth factor beta (TGF β1) in the progression of posthepatitis fibrosis and in the formation and progression of NAFLD in overweight and obese patients have been deepened. The main predictors of progression of hepatic steatosis and fibrosis in comorbidity with overweight and obesity were identified and for their predictionwere constructed logistic regression models. The effectiveness of bicyclol treatment during 12 weeks in overweight patients.
