Essay

The object of the investigation: patients with cancer of the rectum after sphincteropreserving resection, experimental animals after perineal proctectomy. Purpose and methods of investigation: using clinical, morphological (histologic, histochemical, morphometric) and experimental methods of investigation to study the peculiarities of the perineal wound healing in two stages formation of the artificial anus in order to determine the terms of carrying out the second stage of the operation to improve the functional results of the rectal resection for cancer. Theoretical significance and novelty: the peculiarities of the course of the postoperative period and adhesion of the perineal wound, in which the colonic graft has been involved down, has been generalized. The terms of the final stage of the formation of the artifical anus has been established. It has been established morphometric criteria of morphofunctional condition of the excess of the involved down intestine, the zones of coloanal anastomosis,diaphragm of the pelvis, which allows objectively to establish the optimal terms of complesion of the second stage of sphincteropreserving resections, it has been observed the correlation between timely formation of coloanal anastomosis and functional results after the operation. Practicall significance: recieved data can be applied for the optimisation of the treatment of postoperative patients of the given category in the condition of both oncologic dispensary and general medical institutions. The subject and inculcation degree: the criteria of the optimal term of carrying out the second stage of sphincteropreserving resections have been inculcated in pedagogical and medical process of the oncological department. Efficacy of the inculcation: acknowledge of new knowledge and the increase of the efficacy of treatment of oncologic patients. Sphere of application: oncologic dispensaries, oncologic departments, surgical departments of the clinics.
