Essay

The dissertation is based on the materials of screening examination of 2668 patients with acute myocardial infarction (MI) There were 463 patients with MI chosen for the study with ST-segment elevation on EKG. The comparative analysis of clinical effectiveness and safety of accelerated regimens of fibrinolythic therapy (FLT) by tissue plasminogen activators (t-PA) and FLT by streptokinase (SK) in combination with sodium dalteparin has shown that FLT t-PA is more effective than FLT SK, based on the incidence of preventing MI complications and on the 30-day mortality index. FLT t-PA is more safe than FLT SK. The method of combining accelerated (60 min) FLT, 100 mg t-PA with ACT sodium dalteparin (iv bolus 10 000 units before initiating FLT, then in 3 hours switch to subcutaneous injections with the dose of 120 IU/kg every 12 h over the course of 5 - 7 days) - is the optimal method.
