The object is 203 patients on the chronic pyelonephritis; the aim is to develop the criteria of prognosing of clinic tendency and to improve the complex therapy of the chronic pyelonephritis; the methods are clinical, instrumental, biochemical, immunological; the new approach is firstly to disclose the presence of the endothelial dysfunction, obvious changings of acute-phase proteins, the humoral section of immunity and to substantiate the expediency of using both of ofloxacin, biosporin and enalapril in the treatment of the chronic pyelonephritis without or with arterial hypertension; the results are the developing of new chart to get the stable remission of the desease and to prevent the developing of the chronic renal insufficiency; all this thesis have been introducing into training process and practical medicine; the field of medicine
