Essay

The object - patients with cancer of ampullar portion of rectum with unfavorable factors for colon anastomosis application as well as patients in whom symptomatic interventions were performed before; the purpose of research - to elaborate measures for expansion of indications for sphincter-preserving operations; metods of examination - endoscopy, x-ray, ultra-sound, CT, clinical examination; novelty - anatomic factors which may interfere with graft-forming and descending of colon to perineum were defined and studied; possibility of vitable graft-forming in patients with unfavorable factors was proved; possibility of sphincter-preserving operations after previously performed symptomatic surgical interventions was shown; possibility of graft-forming of previously colostomied portion of colon was proved; theoretical and practical results - application of elaborated methods of graft-forming allowed to diminish rate of descended colon necrosis to 4,9%, to widen indications for sphincter-preserving operations. to improove quality of life, to provide social rehabilitation of those patients; the introduced method was embedded in Donetsk Anticancer Center, Mariupol oncological hospital; the sphere of application - medicine (oncology).
