Object - patients with a cancer of a gullet and a cancer proximal departments of a stomach with transition to a gullet; methods - endoscopical, radiological, clinical, laboratory, statistical; novelty for the first time it is established, that indications to use of handsewn or mechanical ways anastomosis should be differentiated: at operations concerning a cancer of a gullet a method of a choice is application of devices for sewing together, at operations concerning a cancer of a stomach with distribution on a gullet - handsewn anastomosis; it is proved, that at operations at patients with a cancer of a gullet use of a handsewn seam raises risk of development of relapse of a tumour by a seam; position is confirmed, that at a mechanical way of formation esophageal anastomosis the risk of development cicatricial stricture anastomosis raises; for the first time the scale of an estimation of quality of a life of patients by which operations with imposing esophageal anastomosis have been executed is developed; Results - it is proved principles of the differentiated application of handsewn or mechanical ways of formation esophageal anastomosis for improvement of results of surgical treatment of patients by a cancer of a gullet and a cancer proximal departments of a stomach with transition to a gullet; branch - medicine.
