3. Object of research – the heavy mechanical trauma accompanying with a shock. Purpose – to improve results of treatment of miners with the heavy mechanical trauma of the support-motor apparatus accompanying with a shock, on the basis of improvement diagnostic and prognosis algorithms, tactics and a complex of medical actions on pre-hospital stage. Methods of researches – clinical, laboratory, statistical, mathematical. For the first time developed of mathematic model of a mine traumatism allowing on the basis of mountain – geological, sanitary-and-hygienic and production factors to predict frequency of traumas on collieries. Adverse influence of factors of mine manufacture on current and outcome of a traumatic shock at miners is established. On the basis of the revealed features of metabolic processes at victims with heavy mine traumas at stages of medical evacuation it is specified them pathogenesis. The method of forecasting of outcomes of traumatic illness on pre-hospital stage at miners with a heavy trauma is developed. The new data on efficiency of the infusion therapies and the antishock pharmacological means applied at injured miners under the ground are established. Specifications of volume and qualitative structure of medical aid are determined is direct on a place of a trauma and at stages of medical evacuation. For the first time on the basis of the account of biorhythms of miners the method of preventive maintenance of a traumatism which consists in elimination of the person of production in case of their adverse connection is developed. Working classification of mechanisms of trauma is developed also. The medical program of rendering of medical aid to injured miners on the basis of prognosis criteria of weight of a condition and outcome of traumas is developed of varied cure program. More perfect complete set of organic equipment is proved. The carried out estimation of efficiency of medical actions - immobilization, analgesics, local anesthesia, infusion therapies - on the offered method hasallowed to develop of pathogenesis the proved complex of medical actions on pre-hospital a stage, allowed to lower hospital mortality for the 1,88 times. The basic conclusions and recommendations have found application in work of medical service of mine-rescue parts, in work of the specialized antishock brigades, resuscitation, and also at teaching of extreme medicine.
