Essay

In clinical part of study of patients with solar retinal burns were revealed main symptoms: visual acuity decreasing, central scotoma, metamorphopsia, acquired dyschromatopsia, depression of contrast sensitivity on high spatial frequencies, decreasing of electro-physiological data, local retinal damage, FAG (focal hyperfluorescence) and OCT (desorganisation of photoreceptor layer and pigment epithelium defect) changes and their dynamics according to different stages. In experimental morphological part of the study were revealed signs of destruction of elements of chorioretinal complex: destroying of disks, delamination and lysis of lamellar structures of photoreceptors, appear of myelin figures in Muller's cells cytoplasm, vanish of melanosomas in pigment epithelium, widening of interendotelial joints of choriocapillars. These findings are indicative of pathogenic action of solar light on the structures of chorioretinal tissue complex. Peculiarities of compensative processes in cell elements of chorioretinal complex after antioxidant application were revealed. Lipin significantly weakens the evidence of structure changes of photoreceptors and pigment epithelium cells. Morphological signs of retina changes due to application of Lipin signs about its protective effect on cellular energy apparatus, enhancement of the processes of transendothelium transport and suppressive effect on the peroxide lipid oxidation. Evaluating of proposed method of treatment of retinal burns with Lipin application primarily proved in experiment, was established its positive influence on the clinical-functional state of retina. The treatment favored to recovering of visual acuity, reduction of central scotomas, diminution of local hyperemia and edema of macular and paramacular zones. So, usage of Lipin in complex treatment, especially in early stages of retinal burn, allows to improve functional rehabilitation of patients with solar retinal burns.
