Essay

The dissertation is devoted to study the peculiarities of arterial hypertension, cerebral hemodynamic and autonomic nervous system status in hypertensive patients with pathology of extracranial arteries with the aim of improving their treatment and prognosis. It was revealed that patients with brain hypoperfusion, in 22% cases characterized by pathological blood pressure profile type "night-peaker". They also have increased arterial wall-thickness associated with endothelial dysfunction. Blood pres-sure levels and heart rate variability depend on localization of stenotic lesions in ver-tebral or carotid arteries. Patients with extracranial arteries stenotic lesion in 68,3% cases characterized by autonomic nervous system dysfunction with hyporeactivity both sympathetic and parasympathetic parts and heavy heart rate stabilization. In hypertensive patients without stenotic lesions autonomic reactivity is increased or normal. The results of this study demonstrate antihypertensive effects of perindopril andamlodipine and its influence on cerebral hemodynamic in hypertensive patients with different level of brain perfusion.
