The dissertation is devoted to optimizing the diagnosis of osteoarthritis (OA) with pancreatic functional insufficiency in conditions of comorbidity based on a comprehensive assessment of the clinical course, destructive-inflammatory, anabolic-compensatory parameters, integral indicators of nutritional status, immunogram, colon microbiome and life quality of patients, as well as predicting the presence of exocrine pancreatic insufficiency (EPI) in patients with osteoarthritis.The structural and functional state of the pancreas in OA was studied. The presence of functional insufficiency of the pancreas in patients with OA and its relationship with the parameters of basic pathogenetic syndromes and nutritional status was established. For the first time, the presence and depth of EPI in patients with OA, depending on comorbid nosologies, was proved. New data were obtained on the relationship between the parameters of comorbidities and indicators of OA in the studied patients with comorbidity of OA and diseases accompanied by EPI. Predictors of the formation of EPI in patients with OA were identified: the total WOMAC index was more than 43, and the Lequesne index was more than 6. Prognostic model of faecal α­elastase content in patients with OA was created and validated, taking into account the following clinical and pathogenetic predictors: total WOMAC index, NSAID index, duration of OA, total GSRS scores, and Lequesne index.
