Essay

Dissertation work is devoted to the complex research of different aspects of the diagnostic and surgical treatment of congenitally corrected transposition of the great arteries (CCTGA). 77 patients with CCTGA entered in the explored group, observed in Amosov Institute of cardio-vascular surgery Academy of medical Sciences of Ukraine in period from July 1987 to December 2003. The mean age of patients was 13,11± 4,7 years (range from 3 months to 47 years). In 77 inspected patients different types of surgical interventions were performed: 60 (77,9 %) patients were treated operatively. Among them in 5 (8,33%) patients surgical treatment was carried out in stage-by-stage technique. In 55 (91,67%) patients radical correction of associated defects was performed. Hospital mortality in analyzed group was 18,3% (11 patients). The main reasons for unsatisfactory immediate and long-term results of surgical correction of associated defects at CCTGA consist in the anatomic structure of LAVV and morphology of the right ventricle, that have to function as systemic ones, that is unusual to them. More perspective can appear the anatomic correction of CCTGA (double switch) or transplantation of the heart.
