3.Object of the study: Type 1 and 2 diabetes mellitus compli-cated with diabetic nephropathy with sufficient renal function. Purpose of the study: To improve the quality of diagnosis of early stages of DN and to work out prognostic criteria of the course, to reveal new links in the pathogenesis of chronic renal failure in diabetic nephropathy. . Methods of research: General clinical laboratory and instru-mental studies, indirect solid-phase immunoenzyme analysis, photometry, quantitative colorimetry, photoelectrocalorimentry, kinetic method, dynamic excretory kidney scan. Theoretical and practical results: The obtained findings allow to determine the markers of early pre-albuminuric stages of DN (fermenturia, increased b2 -microglobulin excretion, reduction in titred acids excretion) which can be used in screening of diabetic microangiopathy. Marked fermeturia, disturbance of acid secretion and b2 -microglobulin excretion are prognostic criteria of DN progress and predictors of unfavorable immediate and long-term periods. Novelty: For the first time, it was determined that the obtained findings allow to determine the markers of early pre-albuminuric stages of DN (fermenturia, increased b2 -microglobulin excretion, reduction in titred acids excretion) which can be used in screening of diabetic microangiopathy. Marked fermeturia, disturbance of acid secretion and b2 -microglobulin excretion are prognostic criteria of DN progress and predictors of unfavorable immediate and long-term periods. One of the compensation factors is calcitonin increase in the blood. Four pathogenetic variants of calcium metabolism disturbance in chronic glomerulonephritis were distinguished. They can be used for scientific substantiation for the disturbance correction.
