Georges I. Surgical treatment of median nerve traumatic injuries on various levels.

The dissertation is devoted to the issues of diagnostics and treatment optimization in patients with traumatic injuries of median nerve on various levels. The work is based on the study of clinical manifestation and clinical course, as well as on the results of surgical treatment in patients with traumatic injuries of median nerve (217 patients) in depend on injury level, character and term of trauma. We developed and implemented in practice diagnostically and treatment algorithms. Indications and contrindications, the term of surgical treatment were defined. The results of surgical treatment were analysed. Reconstructive operations on median nerve with corrective orthopedics ones to combine expediently in injuries of median nerve principal trunk with muscle branches on the upper third of forearm level for the prevention of neurogenic deformation.Pain syndrome is occured in traumatic injuries of median nerve in 53,0%, complex regional pain syndrome - 7,4%. The higher level of injury the more frequently pain syndrome. It is found statistically significant correlation between positive results of surgical treatment and the terms of the trauma. Longtime postoperative electrostimulation serves the decrease of pain syndrome intensity for a long term. The using of our algorithms permits to achieve positive status of upper extremity in 91%.
