We researched medicamental way of pupil dilatation in patients with normal anatomy and function of the iris and in patients with low diaphragmal function of the pupil. Special pattern for achieving maximal medicamental mydriasis before an operation in standard and complicated situations was elaborated. The role of injection of 0.5% solution of lidocaine into the anterior chamber of the eye in achieving and maintaining mydriasis while phacoemulsification as well as its influence on the function of the cornea after the operation were studied. Also we worked out medicamental method of maintaining mydriasis during cataract extraction. As a result of the study new authentic method of mechanical dilatation of the pupil which implies protection of the iris edge (pupiloprotector) was suggested. The effectiveness of this method was compared to the other existing methods. The scheme of achieving maximal mydriasis during cataract extraction, which includes the criteria and consequent stages of inducing and maintaining maximal medicamental mydriasis before operation in standard and complicated cases followed by methods of mydriasis induction and maintaining during an operation as well as methods of mechanical mydriasis with the consideration of the level of trauma possibility they may cause was presented in the result of investigation.
