Thesis for scientific degree of Candidate in Medical Sciences in speciality 14.01.22 - Stomatology. - Kyiv Medical Academy of Postgraduate Education named after P.L.SHUPYK, Kyiv, 2005. The dissertation is dedicated to the questions of diagnostics, investigation of the condition, treatment and prophylaxis of intolerance to metal alloys of dental prosthesis in orthopaedic patients with defects of teeth and teeth rows. Analysis of clinical material had been based on the grounds of examination and treatment of 179 dental patients, including 149 patients of experimental group with intolerance to metal alloys of dental prosthesis at age from 20 to 88 years old, and 30 persons of control group. Correlation between patients has formed 1:4 in favour of female. The main amount of female persons, was at age 41-50 years old (35,0%), but male - 51-60 years old (7,4%); that, as on our opinion, is connected with sexual difference in age realignment in organism both female and male. The control group of the investigation formed 30 patients without sign of intolerance to metal alloys of prosthesis of the same age and sex, which was portioned on two subgroups. 11 female persons (36,5%) and 4 male (13,5%) formed first subgroup, they applied for prosthesis for the first time and had no metallic inclusions in oral cavity. The second subgroup formed 11 female (36,5%) and 4 male (13,5%), who need additional prosthesis on background of already previously installed prosthetic devices. To experimental group patients were enrolled at conditions of presence of metallic inclusions in oral cavity (fillings, crowns, bridges), as possible potential factor of arising the complications. From 528 explored metallic inclusions 478 un. were made from unprecious alloys and 50 un. - from precious alloys, in correlation nearly 10:1, in favour of unprecious alloys. The quite number of crowns (100 un., 43,1%) and stamped-soldered bridges (150 un., 50,6%) were made from steel 12Х18Н9Т with nitride titanium covering MZP - composite YUVAM-11. The offered complex of advanced special methods of investigations of patients with intolerance to metal alloys of dental prosthesis and the sequence of its installation, that has allowed to fix the most typical and informative differential-diagnostic criteria of the clinical forms of intolerance to metal alloys of dental prosthesis, set the diagnosis, develop scheme of medical-preventive measures. For the first time necessity of detaching the fifth clinical form of intolerance to metal alloys of dental prosthesis - combined form was settled down. Differential-diagnostic signs of which were determined by complex clinic-laboratory investigations. Algorithm for medical-preventive measures undertaking had been developed and offered for combined form of intolerance. The rational algorithm of treatment of dental patients with different forms of intolerance to metal alloys of dental prosthesis had been developed. The remote results of such complex treatment of 149 patients have demonstrated the positive effect in 98,0% events. Usage of advanced by us methods of prognosis of intolerance to metal alloys of dental prosthesis, allow to reduce vastly the amount of complications among patients with intolerance to metal alloys of dental prosthesis. The remote results of its use among 179 patients, who were under investigation, give positive effect in 176 cases (98,3%).
