Kazmiruk O. The combined operations in treatment of spreaded ovarian cancer.

Object of work is patients with III - IV stage ovarian cancer. The aim was to expand indications for combined operative interventions in patients with the widespread ovarian cancer by development of diagnostic algorithm. Methods and the equipment - clinical and cytologic examinations; ultrasonic research, MRI, immunologic tests, method of construction of survival tables. Results - on the basis of MRI data the diagnostic algorithm was developed for the most adequate definition of indications and volumes of the combined operations which has allowed to reduce in 13 times diagnostic mistakes at definition during preoperative stage of a of tumor spreading degree. Novelty - advantage of algorithm (clinical examination + ultrasound examination + MRI) for the first time is shown in comparison with results of ultrasound for an estimation of tumor spread in patients with ovarian cancer; advantage of the combined operative interventions that was reflected in decrease of tumor progression and increase of a medianof a survival in patients with III-IV stage ovarian cancer is revealed; for the first time the comparative analysis of the data received at studying of direct and remote results of different variants of treatment of such patients is carried out; positive dynamics of parameters of immunity and their approximation to norm after the combined treatment is revealed. Results were introduced into clinical practice of Kharkiv regional clinical oncologic clinic, the Kirovograd regional oncologic clinic, Institute of medical radiology of AMS of Ukraine.
