Goryachev A. Pulmonary artery trunk grafting in the surgery of congenital heart lesions

Dissertation is dedicated to the study of peculiarities and to the surgical treatment of congenital heart lesions accompanied by the infringement of anatomical connection between venous heart ventricle and pulmonary artery and also to the elaboration and improvement of the grafts (conduits) of pulmonary artery trunk capable to preserve their initial properties for a long period of time. It was found that pulmonary artery trunk grafting is indicated for the creation of a single or parallel outflow tract from the venous heart ventricle and that it is an effective method of the surgical treatment of a complex congenital heart lesions, which are accompanied by the breach of anatomical connection between the venous ventricle and the pulmonary artery. A new constructions of pulmonary artery trunk grafts were elaborated and introduced into clinical practice. A comparative evaluation of a different grafts function in the remote period was done. Immediate and remote results of the surgical treatment of a different congenital heart lesions were studied depending on the type of pulmonary artery trunk grafts and on the form of congenital heart lesions. Autopericardium and the pulmonary artery homograft are the materials capable to provide a full value functioning of the conduit no less than 10-12 years.
