Rizk G. Surgical treatment of infective endocarditis, complicated by heart abcesses.

Dissertation is dedicated to the problems of diagnostics and of surgical treatment of infective endocarditis, complicated by heart abscesses. As the result of the study, conducted on the base of the treatment of 114 consecutive patients with heart abscesses as complication of infective endocarditis and of 360 patients with infective endocarditis not complicated by heart abscesses, a clinical picture and differential diagnostic signs between the two groups were described. The group of symptoms and of signs were found, which permit to suspect the development of a heart abscess and to conduct the transesophageal echocacdiography, as the most informative method in the diagnostics of heart abscesses and to fulfill an early surgical intervention aimed to prevent further destruction of heart tissue, to decrease the risk of operation and to improve its results. Indications to operation were determined. Complex of measures were elaborated in order to make the intervention a radical one: methods of an abscess elimination, of the treatment of heart abscess cavity and of the heart chambers with the use of the solution of antiseptic, methods of a heart abscess cavity plasty, methods of plasty of destructed valves fibrouse rings, methods of prosthetic replacement or plasty of heart valves and plasty of occurred heart tissue deficiencies; methods of prevention of infective endocarditis reoccurrence were elaborated. System of outpatient rehabilitation and follow up of operated on patients were worked out.
