Bikshaeva Y. Pecularities of the clinical typology and the treatment of somatoform disorders at adolescents with functional disturbances of cardiovascular system.

In 1998-2003 the complex research of 186 adolescents with functional disturbances of cardiovascular system (FD CVS) and somatoform disorders (SFD). At the first stage the clinical typology of SFD was studied, the role of constitutional-genetic, cerebral-organic and psycho-social factors of etyology and pathogenesis in their appearence and clinical pathoplastic was researched. At the second stage during 16-weeks, 2-phases comparable research the effiency of psychotherapy, cardiotroptherapy and differential psychofarmatherapy in SFD treatment at adolescent with FD CVS was studied. Outcome of research: SFD in adolescent age are non-homogeneous both from etyopatogenetic conceptualization and from the view of the clinical typology. They consist heterogenic group of psychic disorders, which creates continuum from relatively simple by their mental structure of somatoform vegetative disfunctions to polymorph psychopathological formations with different additional symptoms. The complex treatment of SFD on adolescents with FD CVS was developed. The treatment includes etyopathogenic substantiation, sequence, differentiality and dynamics of therapeutical impact. Indications to differential pharmacotherapy with SISR (Sertralin), neurometabolical medications (Instenon), mood stabilizer (Lamotridgin) were scientifically validated. Pharmacotherapy should be applied in case of insufficient efficiency within 8 weeks of a complex psychotherapy or cardiologic treatment.
