Object of the study: Serous benign, terminally malignant and malignant tumors of the ovaries. Purpose of the study: To discover the role of morphological and immunohistochemical characteristics of the stromal-vascular com-ponent of benign, terminally malignant and malignant serous tumors of the ovaries in evaluation of differentiation degree of these tumors. Methods of the study: Macroscopy, organometry, histology, histochemistry, morphometry, immunohistochemistry and statistical methods. Clinical analysis of the case histories with the study of epidemiology and clinical features of serous ovarian tumors of various degree of differentiation. Practical significance of the obtained findings: The obtained findings about collagen formation, ratio of GAG fractions, ratio of fibroplastic cells, the degree of expression of receptors to endothelin-1 can be diagnostic criteria of the degree of connective tissue stroma maturity in the serous tumors of the ovaries. The use of these criteria will allow prognosisa possible progression of the tumor, to make differential diagnosis of serous benign, terminally malignant and malignant tumors of the ovaries. Novelty of the obtained results: The performed investigation allowed to reveal differential diagnostic and prognostic stromal-vascular criteria of serous ovarian tumors of various degree of maturity (serous benign, terminally malignant and malignant). The ideas about the role of fibroplastic cells - fibroblasts and fibrocytes in maturation of the connective tissue stroma in the ovarian tumors were expanded. For the first time, it was shown that impairments of fibroblast population maturation and increased production of non-sulfated GAG and reduced synthesis of interstitial collagen I with increase of collagen III synthesis are present in the connective tissue stroma of benign tumors when compared with the stroma of the healthy ovaries. The endothelium of the stroma vessels expresses receptors to endothelin-1. The basal membrane of the epithelium and vessels contains collagen IV. The amount of collagen IV decreases in the basal membranes of the vessels in malignant tumors. The obtained findings were used to show that morphogenesis of the stroma in the serous tumors of the ovaries developed similarly to the organs with the participation of fibroblasts, fibrocytes and GAG. For the first time, it was established that type IV collagen appeared in the stroma of serous cancer, which promoted proliferation and migration of the cellular elements of its surrounding, that is can play a role in the tumor progression. Introduction: The obtained findings were introduced in the work of pathological anatomy department of Kharkiv State Medical University and Kharkiv Medical Academy for Post-graduate Training. The findings of the research were introduced to the practice of pathology departments of Kharkiv regional Clinical Hospital, Kharkiv City Hospitals No. 1, 2, S.Grigoriev's Institute for Medical Radiology. Sphere of application: medicine, pathological anatomy.
