Essay

Object of research: children of early age, patients with salmonellosis, shigellosis and escherichiosis. The purpose of research: improvement of diagnostics, forecasting of clinical course and treatment of children of early age sick of salmonellosis, shigellosis, escherichiosis on the basis of research of the features of the structurally functional condition of a liver and portal hemodynamics of patients. Methods of research: clinical, bacteriological, serologic, instrumental, and statistical methods of research are used in the work. Theoretical and practical results: the results of the work expand an intellectual scope of the practical doctor about pathogenesis of salmonellosis, shigellosis, escherichiosis of children of early age and allow him, basing on results of ultrasonic research of patients' liver to approach to an estimation of heaviness of pathological process and to improvement of rehabilitation actions more objectively. To practical public health services accessible, noninvasive, high-quality informational method of forecasting of clinical course of the mentioned intestinal infections on the basis of simple summation of diagnostic factors of value of the clinico-instrumental data which were revealed in the debut of illness is offered. It allows the doctor of a practical link at the initial stages of illness, in case of forecasting its adverse clinical course, to carry out medicamentous correction of pathological process in due time. Besides, the use of vasoactive medications, in particular of pentoxyphillini, in complex therapy of salmonellosis, shigellosis, and escherichiosis in diagnosing of their possible adverse clinical course is recommended that allows to lower percentage of adverse variants of clinical courses of the specified intestinal infections. Novelty: on the basis of complex clinico-laboratory-instrumental researches of children sick of a salmonellosis, shigellosis and escherichiosis, the determining role of etiological factor in formation of a clinical picture of illness and absence of its value in changes of the structurally functional organization of a liver and a condition of portal hemodynamics is proved. For the first time the major importance of a portal blood-groove in formation of the structurally functional organization of a liver under intestinal infections is revealed, that, as a whole, reflects the heaviness of pathological process and opens ways to increase objectivity of an estimation of heaviness of disease. The expediency of use and high therapeutic value of pentoxyphillini in complex therapy of children sick of intestinal infections is scientifically argued and practically proved. Degree of introduction: the results of the carried out researches are introduced into practice of regional children's infectious clinical hospital of Kharkov, children's polyclinic of Kharkov, regional children's hospital of Lvov, regional children's hospital of Chernovtsy and is also used in educational process of faculties of children's infectious diseases of the state medical universityof Kharkov, the medical academy of postgraduational education of Kharkov, the national medical university of Lvov.
