Zhovnir V. Correction of a low cardiac output syndrome in the early postoperative period in patients with critical congenital heart lesions

Dissertation is dedicated to the study of hemodynamical peculiarities in a critical congenital heart lesions and to the determination of the causes of a low cardiac output syndrome, to the improvement of existing methods and to the elaboration of new effective methods of correction of this syndrome in an early postoperative period in patients with this grave pathology. It was found that low cardiac output syndrome development is the main reason of unsatisfactory results in the critical congenital heart lesions surgical correction. A new important complex methods of low cardiac output syndrome correction were elaborated during the study. These methods are based on the improvement of a fetal circulation ways functioning in the preoperative stage and on the regulation of the level of preload, afterload and myocardium inotropic state in an early postoperative period. Comparative evaluation of the standard and of the improved methods of low cardiac output syndrome correction in patients with critical congenital heart lesions was performed. The results of correction of a different types of critical congenital heart lesions were studied. Efficiency of improved methods of low cardiac output syndrome correction in all types of critical congenital heart lesions was proved. Mortality in the early postoperative period was lowered on account of the improved low cardiac output syndrome correction methods application.
