Adamchuk G. Transperitoneal resection in cancer of the proximal portion of the rectum: indications, results of treatment, prognosis.

Object - patients with cancer of proximal section of rectum, to whom the transperitoneal resection was done with the formation of duplicate colorectal anastomosis. Aim - to develop the reliable and functionally full method of surgical cancer treatment for the proximal rectum section. Methods - clinical, biochemical, morphological, instrumental (endoscopic, roentgenological, ultrasonic, computer and magnetic-resonance tomography), statistical. Novelty - it was proved the advantage of the developed procedure of the transperitoneal resection of a rectum with the use of the duplicate anastomosis, which allows to restore the continuity of a rectum at different diameters of connected sections, including the patients with the complicated cancerous process. Results - the proposed procedure for the restoration of the intestinal tube continuity by means of formatting the inter-intestinal colorectal duplicate anastomosis permitted to considerably approve the direct results for treatment of the patients with thecancer of the rectum proximal section; the proved high reliability of the proposed treatment procedure gave the possibility to extend the indications for the primary restoration of the intestinal tube continuity among the patients of all age groups, that contributed to reduce the frequency of performing the obstructive surgical operations with the formation of unnatural anus; in the remoted period there were noted the satisfactory functional results. The method was implemented in the Donetsk regional anticancerous Center, Oncologic clinics and surgical wards of Dnipropetrovsk, Krivyi Rig, Mariupol. Branch - Medicine (Oncology).
