Essay

Object of research - renewal of spinal support with the use of endoprosthesis. The purpose to improve efficiency of reconstructive-restorations operations on a spine by development of the system of the individual planning of endoprosthesis of interbody support of thoracic and lumbar segments. Research methods are the general clinical, roent-genologic (roentgenometry of spine). geometrical and fi-nite-element modelling (work graphic stations, equipped by the licensed CAD-systems of Pro/ENGINEER, KOMPAS, Inventor and CAE-systems LS-DYNA3D, Pro/MECHANICA, Win Machine, ANSYS), statistical meth-ods New knowledge: On the basis of the conducted retro-spective analysis of results of surgical renewal of anterior| spinal support of thoracic and lumbar localization with the use of existing and common for today implants it is set that principal reason of unsatisfactory results of surgical inter-ferences is disparity of mechanical properties of used im-plants to mechanical descriptions of neurocentrums con-crete patient. First offered approach to research of the difficult bio-mechanics systems, which allows to take into account ge-ometry, physical and mechanical properties of material and condition of co-operation of elements substantially differ-ent nature: mechanical constituent and biological fabrics. First the developed mathematical model of the biome-chanics system "vertebra - endoprosthesis - vertebra''. First the new system of the automated research of the tensely-deformed state of elements of the system offered on principle " vertebra-endoprosthesis" for the design of operations of endoprostheting, which enables it is optimum to pick up the parameters of endoprosthesis and charts of surgical operations taking into account the individual fea-tures of patients. First the got results of numeral design of the tensely-deformed state of neurocentrum with varying of model and properties of material of trabecular bone. Got on principle important result about influence of anisotropy of properties of trabecular bone onthe level of tensions in cork fabric. First the developed and offered original construction of endoprosthesis of spine for renewal of anterior support of his front departments taking into account individual descrip-tions of patient. On the basis of method of mathematical design the of-fered automated system of the individual planning of endoprosthesis of spine. This computer system for the de-sign of operations of endoprostheting spine and research of the tensely-deformed state of elements of the biomechanics system it is optimum to pick up "vertebra-endoprosthesis" possibility parameters of endoprosthesis and charts of sur-gical operations taking into account the individual features of patients. Developed and the rational model of endoprosthesis of spine inculcated in practice of health protection for renewal of spinal support| of his front departments and certain possi-bilities of his clinical use. Developed the dedicated system of the automatic plan-ning and certainly-element design of elements of the bio-mechanics system the "vertebra-endoprosthesis" and method of renewal of spinal support front departments of spine of thoracic and lumbar localization taking into account the individual features of patients is inculcated in Institute of Spine and Joint Pathology named after prof. M. I. Sytenko AMS of Ukraine, in the Regional clinical hospital of Uzgorod and in an educational process and on practical em-ployments on the department of general surgery with the course of traumatology and orthopedic of the Uzgorod na-tional university.
