Essay

The thesis contains a new approach to the questions of early diagnostics of osteoarthrosis against the background of hypermobile syndrome in adolescents. There has been given the definition of different factors of the pathology manifestation, the main of which are osteoarthrosis-burdened heredity, presence of the risk factors combination, and reactive arthritis in anamnesis. The results of the examination of clinical osteoarthrosis manifestations during the debut of the disease in adolescents are given. The peculiarities of the pathology course depending on sex and hypermobile syndrome degree have been investigated. There has been indicated the heterogeneity of articular cartilage lesions during osteoarthrosis related to age and clinical symptomatics duration. There have been detected the changes in metabolism of connective tissue structures such as collagen and proteoglycans, and immunological shifts related to different immunity links including cellular, humoral, monocyto-phagocytic levels. During the investigation of cytokine concentration changes, particularly IL-b, IL-6 and FNP-a in the serum of adolescent patients, the role of inflammatory component in pathogenetic mechanisms of osteoarthrosis against the background of joint hypermobility has been confirmed. The backbone pathogenetic interrelations among immunity parameters and individual connective tissue structures have been analyzed; the role of each of them in osteoarthrosis formation has been established. The criteria of osteoarthrosis diagnostics and differentially diagnostic criteria of the pathology with different articular diseases are described.
